  • Federal or state Operating Authority (MC or USDOT number) for at least 90 consecutive days
  • A or B rating according to Internet Truckstop
  • FMCSA safety rating of satisfactory or N/A
  • Copy of signed W-9
  • Proof of insurance ($1,000,000 auto liability coverage, $100,000 cargo coverage)
  • At least one reference
